MCP Activity log and memory reliability

MCP Activity page now shows all read, write, and auth events with filters, plus memory and import indexing reliability improvements.

  • MCP Activity -- one unified table for read, write, and auth events, with combinable token and event-type filters. Delete remains store-only.
